permission to have fun

For the protection of MY members, students participating in MY activities must have a permission form on file which gives MY staff permission to transport youth and obtain medical treatment in the event of an emergency.

To obtain a permission form, just click on the link below, print the form, complete and email, mail or turn it into the Education and Youth office or Church office.
